Advisor/Major/Graduation Date Change Form

You may use a single form to make any combination of advisor, major, and graduation date changes at the same time.

  • Advisor: Requires the signature of your NEW advisor.
  • Major: If your advisor is in the department of your major, you do not need any signatures. If you are declaring a double major, this is NOT the form you use. Double major applications are available in the Registrar's Office.
  • Graduation Date: No signatures required.

Incomplete Request

Requests for incompletes are handled via petition to the Academic Procedures Committee.  Click here for further instructions on APC petitions.

Independent Study Form

IndependentStudy.pdf (5k)

This form must accompany your pre-enrollment or enrollment for any Pomona independent study course numbered 99 or 199, or any cross-college independent study course. Forms will not be accepted after the tenth day of classes in any semester, and must be submitted with an enrollment or Add/Drop form. You must obtain the signatures of both your independent study instructor and your advisor.

This form should be completed by students requesting a letter of recommendation from an instructor.

Summer Reading & Research Form

This form must be submitted to the Registrar's Office by the end of the semester prior to the desired summer of study. Summer Reading & Research courses are numbered 098 (lower division) and 198 (upper division).

Study must take place under the supervision of a Pomona College faculty member who is teaching on campus in the spring preceding the summer of study, and who will be teaching on campus in the fall semester following.

You must obtain the signatures of your instructor and the department chair in the area of study. Incomplete forms will not be accepted.

    If you have a valid Pomona e-mail address (must end in @pomona.edu or @mymail.pomona.edu), you may request a transcript by sending an e-mail message to registrar@pomona.edu.  Please include the following information:
  • Your full legal name (if you attended Pomona under a different name, please include both your current and former names)
  • Student ID or Social Security number
  • Date of birth
  • Dates of attendance
  • Address to which we should send your transcript

Transfer Credit Approval Form

The Transfer Credit Approval Form should be used whenever you are planning to take courses away from Pomona College for which you wish to receive credit toward graduation. The form may be submitted prior to completion of the work away, or upon return to Pomona. You must obtain an appropriate Pomona College department chair signature for each course for which you are requesting credit. Please read the form itself for complete policy information regarding transfer credit.
