All entering Claremont Colleges Undergraduate/Transfer students have pre-arrival health and immunization requirements that must be completed prior to the start of their first term at The Claremont Colleges. Medical record requirements
- Each student must upload a completed Entrance Physical and Immunization Record from their home provider, previous school, or military service to the *online health portal
- *Access to this form requires your new college credentials.
- NCAA Intercollegiate Athletics are not required to submit an Entrance Physical form to SHS. The documentation you provide for your NCAA Intercollegiate Athletic Clearance will be forwarded directly to SHS by your Athletics Team. Any updates or communication regarding this process will come from your Athletics Team.
- All students must complete a series of online health forms, including a TB Screening Form, accessed through the Health Service student portal.
- Any student who will be under 18 years old on August 15, 2026 must complete the Minor Consent to Treat Form.
- Please allow 7-21 days for your forms to be processed. After processing, you will receive a secure message through the Student Health Portal indicating whether your requirements have been satisfied or if additional documentation is needed
